Parental consent for Minor Child to visit Humane Society of Sedona

I hereby give permission for my child, _________________________________, age, _______, (14 years or older), to visit the Humane Society of Sedona's animal shelter. I understand that children visiting the shelter are allowed to play with the dogs/puppies in enclosed areas, to play with the cat/kittens, and to assist shelter personnel and volunteers with grooming of the animals. I understand that any dogs or cats which are known by shelter personnel to be wild or dangerous will be so labeled and the children informed not to touch any such animals. I also understand, however, the possibility that any animal may, accidentally, playfully, or deliberately, scratch or bite, and that the Humane Society will not be held liable for any injuries incurred.
